Stop Repairing Nervous Systems with Spanners: Why Your Improvement Strategy is Failing

Most business improvement programs fail for a reason nobody wants to admit. Not because the tools are bad. Not because the people are lazy. Not because the strategy deck lacked enough arrows, circles, or phrases like “enterprise-wide alignment.” They fail because the organisation is being treated as the wrong kind of thing.
For more than a century, we have borrowed our improvement logic from the factory. Find the broken process. Map it. Measure it. Standardise it. Automate it. Govern it. Then, surely, performance will improve. It sounds perfectly sensible. Which is precisely why it is so dangerous. The problem is not Lean, Six Sigma, Agile, or AI.
These tools can be extremely useful. But a useful tool applied to the wrong model of reality becomes a very sophisticated way of missing the point. A scalpel is excellent for surgery. It is less useful for making toast.
The Problem: We Are Solving Cognitive-Era Problems with Industrial-Era Instincts
The industrial-era model of improvement assumed that organisations behaved like machines. That was not ridiculous at the time. In factories, work was visible, repeatable, and physical. Variation was often a waste. Compliance created quality. Standardisation created scale. If something went wrong, you could inspect the line, retrain the operator, remove the defect, and improve the output. Splendid.
Then came the software era. The machine metaphor did not disappear. It simply acquired a login screen. Organisations began thinking in workflows, platforms, integrations, backlogs, dashboards, and system architecture. But we are now in a different era altogether. The cognitive era.
In this era, the most valuable work is shaped by attention, judgment, trust, motivation, interpretation, identity, memory, creativity, and social dynamics. Work no longer lives only in processes or systems. It lives inside people’s minds. And minds are not assembly lines.
That is why so many improvement programs look magnificent on slides and faintly absurd in real life. The process map says the work should flow smoothly, but the team is overloaded. The dashboard says the data is available, but nobody trusts it. The operating model says empowerment, but the culture says, “Wait for permission.” The new system says it will save time, but users experience it as one more hungry mouth to feed.

A Better Framework: The Three-Question Test
Before launching another improvement initiative, introducing another platform, or asking teams to “embrace change,” ask three questions.

1. What kind of system are we dealing with?
Is this a predictable system or an adaptive one?
If the work is repetitive, visible, and stable, traditional process improvement may work beautifully. But if the work depends on judgment, interpretation, trust, and human decision-making under pressure, then treating it like an assembly line will create theatre, not transformation.
The first mistake is assuming every business problem is a process problem in a traditional way. Some are. Many are not. Some are cognitive load problems. Some are trust problems. Some are incentive problems. Some are identity problems. Some are fear problems dressed up as governance.
2. What is the current system making rational?
People rarely behave randomly at work. They behave rationally according to the system's real rules. Not the rules written in the policy. The real rules.
What gets rewarded?
What gets punished?
What gets ignored?
What gets someone promoted?
What gets someone quietly excluded from the next meeting?
If people do not use the new system, maybe it adds work rather than removing it. If leaders avoid making decisions, the organisation may punish wrong decisions more visibly than it rewards right ones. If teams resist change, maybe they have watched five previous “strategic priorities” disappear after the executive sponsor moved roles. Resistance is not always defiance. Sometimes it is intelligence. Sometimes it is the organisation telling you, “This does not feel safe, useful, meaningful, or real.”
3. Are we designing for compliance or intelligent action?
Industrial-era improvement often asks, “How do we make people follow the process?” Cognitive-era improvement asks a better question: “What would make the better behaviour feel obvious?” That single shift changes everything. It moves improvement from enforcement to design. From persuasion to environment. From process control to cognitive design. From asking people to try harder to making the right action easier.
Because in human systems, behaviour is not produced by logic alone. It is produced by context. Change the context, and behaviour changes. Leave the context untouched, and your new process becomes another PDF in the corporate archive.
A Practical Example: The Dashboard Nobody Used
Imagine a large service organisation that introduces a new performance dashboard. The dashboard is accurate. It is well-designed. It has been built by competent people. It provides leaders with visibility into key operational metrics. And yet, six months later, almost nobody uses it properly. The traditional conclusion is predictable. “We need more training.” “We need stronger governance.” “We need leaders to hold people accountable.”
Possibly. But the cognitive-era diagnosis asks different questions. Do teams trust the data? Does the dashboard help them make decisions, or does it merely expose them to scrutiny? Are the metrics connected to outcomes that they actually influence? Is the dashboard reducing cognitive load, or adding to it? Does using the dashboard make someone’s work easier, or simply make their performance more visible to people above them?
Suddenly, the problem looks different. The dashboard did not fail because people hate data. It failed because the system treated information as control, not as a decision aid. So the solution is not just better training. It may be redesigning the dashboard around real decisions, simplifying the measures, involving teams in interpretation, creating safe review rituals, and rewarding learning rather than defensive reporting.
That is the difference between tool-first improvement and cognitive design. One installs the dashboard. The other changes the conditions under which the dashboard becomes useful.

The Real Shift
The next generation of high-performing organisations will not be built by abandoning Lean, Agile, Six Sigma, data, automation, or AI. They will be built by placing those tools inside a better model of the organisation.
Lean still matters.
Agile still matters.
Data still matters.
AI will matter enormously.
But they must be applied to organisations as they actually are: human, cognitive, social, adaptive systems. The best leaders will not ask people to behave like predictable components in a machine. They will design environments where people can think clearly, act wisely, learn quickly, and improve naturally.
That is the real shift. From machine logic to human logic. From process control to cognitive design. From improvement as a program to improvement as an organisational capability. The tools were never the real problem. The model was.
And until the model changes, every new improvement initiative risks becoming just another attempt to repair a nervous system with a spanner.

6. Humanocracy, Gary Hamel and Michele Zanini. A strong critique of bureaucracy and an argument for organisations that unlock human initiative rather than bury it under layers of control. It closely connects to the idea that excessive governance can quietly destroy adaptability.
7. Nudge, Richard Thaler and Cass Sunstein. A useful guide to choice architecture and behavioural design. It shows why small changes in defaults, context, timing, and framing can shape behaviour more effectively than persuasion or enforcement.
